What is the ACL?
The anterior cruciate ligament (ACL) is one of the main ligaments inside the knee. It runs from the thigh bone to the shin bone, stops the shin from sliding forward and keeps the knee steady when you twist, pivot or land.
When it tears, the knee can feel loose or give way, and it becomes harder to trust on stairs, slopes and uneven ground. Dr. Singh treats ACL injuries as part of sports injury care and knee problems in Jamshedpur.
How does an ACL injury happen?
Most are non-contact injuries
Most ACL injuries happen without a direct blow. They occur when you suddenly change direction or land awkwardly from a jump, so the knee twists while the foot is planted.
Body mechanics matter
Poor knee alignment and muscle imbalance can add stress on the ACL. Movement patterns and muscle weakness make it more vulnerable, and proper training to correct them can reduce the risk. Women are at higher risk of ACL injury than men.
Signs to watch for: the knee gives way, swelling soon after a twisting injury, and a feeling of instability, especially when walking downhill or coming down stairs.
How is an ACL injury diagnosed?
Diagnosis rests on a careful clinical examination of the knee and an MRI. The examination shows how stable the knee is, and the MRI shows the tear itself and any injury to the meniscus or cartilage, which often occur together.
Please bring any earlier X-rays or scans to your visit. You can also read all about ACL tears on our blog.
ACL tear treatment in Jamshedpur: do you need surgery?
Not every ACL injury needs an operation. The advice depends on the grade of the tear and on how the knee behaves when examined.
Surgery is not needed. You are advised exercises and follow-up.
The examination decides. If the ACL is not competent, you may be advised surgery. If it is competent and working, you are advised exercises and follow-up for a few months.
Surgery is advised.
My knee feels fine now. What next?
A few weeks after the injury, many people have no pain and walk normally. But ACL surgery is not done for pain; it is done to restore stability. A knee without a working ACL is more prone to meniscus and cartilage damage a few years down the line. Both are painful and can lead to early osteoarthritis. Read do you really need ACL surgery if your knee feels better.
What if I do not have surgery?
You carry a higher risk of meniscus and cartilage injury, and of developing osteoarthritis earlier in life.
Does the ACL heal by itself?
Usually not. Spontaneous healing has been seen in low-grade tears and in younger people, but research is still ongoing. For now, the global consensus is to replace the ACL rather than repair it.
ACL reconstruction in Jamshedpur
ACL reconstruction is the widely accepted standard treatment for a torn ACL. It is an arthroscopic (keyhole) procedure in which a new ACL is made from a graft, usually taken from the same leg. Arthroscopy uses a camera and fine instruments, so the joint is not opened up.
Hamstring graft
Tendons from the back of the thigh (semitendinosus and gracilis), a commonly used graft that suits all-inside reconstruction.
Peroneus longus or donor graft
Considered when the other graft options are not suitable, based on your knee and the injuries found.
When the knee needs extra stability for demanding sport, LET and internal bracing can be added. Meniscus or cartilage damage is dealt with in the same surgery. See what happens during ACL reconstruction.
Recovery after ACL surgery
Recovery is gradual and depends on strong physiotherapy support and on whether other injuries were treated at the same time.
Walking is usually allowed.
Return to office work, with a brace.
Return to sport can begin with strong physiotherapy support.
Most people are advised a 1-year return-to-sport programme.
The steps and timing are explained in recovery and rehab after ACL reconstruction. Your own timeline will be set at your reviews.
What does ACL surgery cost?
The cost goes up if another injury needs fixing or if newer procedures are added, and it varies with the hospital and implants. We discuss cost upfront at your consultation. Patients with insurance or a government health card may be covered under a fixed package.
Can ACL surgery fail?
Failure is rare, but it is seen around the world. The usual causes are:
- Improper graft fixation or implant failure
- Infection
- Returning to sport too early
- Improper or incomplete rehabilitation
If an earlier ACL surgery has failed, a repeat (revision) surgery is needed. It requires extensive imaging for planning and implant selection, and the recovery is longer.

ACL questions, answered
Does an ACL tear heal on its own?
Usually not. Spontaneous healing has been seen in low-grade tears and in younger people, but research is still ongoing. For most complete or high-grade tears, the global consensus is to replace the ligament with a graft rather than repair it.
Do I need surgery for an ACL injury?
It depends on the grade of the tear and on how stable your knee is. Low-grade (grade 1) tears do not need surgery. For a grade 2 tear, your examination decides: if the ACL is not competent you may be advised surgery, and if it is working well you will be advised exercises and follow-up for a few months. High-grade partial or complete tears are usually advised surgery.
My knee feels fine after the injury. Do I still need surgery?
Many people feel pain-free and walk normally a few weeks after the injury. ACL surgery is done to restore stability, not only to relieve pain. A knee without a working ACL is more prone to meniscus and cartilage damage a few years down the line, which is painful and can lead to early osteoarthritis. This is why a check-up and MRI are worth doing even when the knee feels fine.
What happens if I do not get ACL surgery?
You have a higher risk of meniscus and cartilage injury and of developing osteoarthritis earlier in life. If you play pivoting sports or the knee keeps giving way, the risk is higher still.
How long is recovery after ACL surgery?
Walking is usually allowed within the first few weeks, and many people return to office work by about 6 weeks, with a brace. With strong physiotherapy support, return to sport can begin from about 6 months, but most people are advised a 1-year return-to-sport programme. The exact duration depends on whether other injuries were treated during the same surgery.
What does ACL surgery cost?
A basic ACL reconstruction typically costs about ₹80,000 to ₹1.2 lakh. The cost can be higher if another injury needs fixing, if newer procedures are added, or depending on the hospital and implants. Costs are discussed upfront at your consultation, and patients with insurance or a government health card may be covered under a fixed package.
Can ACL reconstruction fail?
Failure is rare but is seen around the world. The usual causes are poor graft fixation, implant failure, infection, returning to sport too early and inadequate rehabilitation. If an earlier ACL surgery has failed, a repeat (revision) surgery is needed, with detailed imaging for planning and implant selection, and a longer recovery.
